Occupational Therapist Job Summary:
The Occupational therapist uses purposeful activities or interventions to maximize the independence and the maintenance of health of any individual who is limited by a physical injury or illness, a cognitive impairment, a psychosocial dysfunction, a mental illness, a developmental or learning disability, or an adverse environmental condition. The occupational therapist uses skilled observation or the administration and interpretation of standardized or non-standardized tests and measurements to identify areas for occupational therapy services.

Occupational Therapist Essential Functions and Responsibilities:
1. The assessment, treatment, and education of or consultation with the individual, family, or other persons
2. Interventions directed toward developing daily living skills, work readiness or work performance, play skills or leisure capacities, or enhancing educational performance skills
3. Providing for the development of: sensory-motor, perceptual, or neuromuscular functioning; range of motion; or emotional, motivational, cognitive, or psychosocial components of performance

Occupational Therapist Qualifications:
Must possess current state license in good standing with the State Board
Interpersonal skills necessary to Communicate Accurately and Precisely to Patients and Family Members
Experience in a Skilled Nursing Facility Preferred

Assisting residents with ADLs, and work with the team to ensure appropriate resident care is provided

Assisting at meals

Following infection control and center policies

Measuring and recording heights and weights temperatures, pulses, respirations and blood pressures (TPRs and B/P) as instructed

Reports all changes in residents’ condition to supervisor as soon as practical

Certified Nursing Assistant (CNA) QUALIFICATIONS:

Physical Therapy Assistant ( PTA ) Job Summary:

The Physical Therapist Assistant ( PTA ) performs patient related activities under the direction of a physical therapist. Physical Therapy Assistant ( PTA ) Major Duties and Responsibilities:

Assisting the physical therapist when performing tests, evaluation and complex treatment procedures
Observing, recording and reporting to the therapist the condition, reactions and responses related to assigned duties
Assisting the therapist in consulting with patients, family members and other health care professionals as appropriate
Assisting with the care of the physical therapy department and equipment
Assisting the physical therapist with in-service programs and other meetings as assigned

Duties & Responsibilities

Purpose of Position:

This class of positions encompasses professional, supervisory or administrative work of varying degrees of difficulty and responsibility in the operation of a social work department providing psychosocial and other social work services to different types of patients across various settings. Ensures patient-centered quality care and services in support of better outcomes and improved patient experience, in accordance with all applicable local, state, and federal rules and regulations, and social work standards of care. There are five (5) Assignment Levels within this class of positions; all personnel perform related work.

Assignment Level II Under general supervision, with some latitude for the exercise of independent judgment, in addition to performing the typical tasks described in Assignment Level I above with or without supervision based upon licensure and at a higher level, performs the following typical tasks:

Examples of Typical Tasks:

1. Mentors and assists less experienced social work staff in social work techniques and serves as a resource in various aspects of advanced social work practice.

2. May supervise graduate and undergraduate social work students, as well as volunteers and other auxiliary personnel, as needed.

3. May assist in the provision of clinical services only under supervision of a New York State LCSW, licensed psychologist, or psychiatrist or perform such functions independently as a LCSW.

Minimum Qualifications

1. Valid license as an LMSW issued by the NYSED; or

2. Valid license as an LCSW issued by the NYSED.

Under general supervision, serves as a licensed, independent practitioner responsible for planning,
coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures

SUMMARY OF DUTIES AND RESPONSIBILITIES

" Provides continuous comprehensive medical and nursing care, including preventive and supportive
" health care services to patients and families.
" Manages health problems and coordinates health care for various patient populations. Identifies and
" diagnoses illnesses and physical conditions and implements therapeutic and corrective measures.
" Consistent with the established nursing practice, differentiates between normal and abnormal
" findings in the screening process to detect changes in health status that require physician s
" intervention.
" Develops and maintains partnerships with community-based organizations and makes referrals to
" consulting physicians and/or other health care professionals and community agencies, as necessary.
" Evaluates and interprets patient data, in order to plan and implement appropriate nursing
" interventions.
" Consults with physicians and other health care practitioners in the evaluation of health screening
" findings to make prospective decisions about treatment that will best support improved outcomes
" and patient satisfaction.
" Formulates differential diagnosis and the plan of care based on medical history, physical exams,
" nursing assessment and other pertinent information. Identifies health and medical risks and needs.
" Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as
" per clinical standards and practice protocols, where needed, to achieve medical and health goals.
" Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and may prescribe
" appropriate pharmacological and non-pharmacological interventions and therapies.
" Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled
" visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal
" availability and utilization of appropriate resources.
" Provides education, guidance, and direction to nursing staff and other members of the health care
" team to ensure quality and safe medical and nursing care is provided.
" Provides education and counseling to patients and families regarding health status, treatment and
" progress, medication management, disease prevention and health promotion, records and maintains
" a system of relevant documentation.

ORGANIZATIONAL RELATIONSHIPS:
" Report clinically to the Department Chairperson and the designated supervising physician(s).
" Report administratively to the Chief Physician Assistant and/or designee.
" Work collaboratively with representatives from dietary, nursing, respiratory, social services and
other patient care/support service departments.

Minimum Qualifications

1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and
2. Current certificate as a Nurse Practitioner issued by the New York State Department of Education
(NYSED); and
3. A current certification issued by the State Education Department to prescribe medication.

For Assignment to Nurse Practitioner (Primary Care/Behavioral Health)
1. In addition to meeting the qualification requirements of Nurse Practitioner, must be assigned to a Primary Care or Psychiatric/Behavioral Health setting
